Job summary

Iowa Home Care is seeking a compassionate Physical Therapist Assistant to provide skilled in-home therapy in Webster City, IA. You will work under the supervision of a licensed Physical Therapist to improve mobility and safety for patients in their homes.

You will collaborate with a multidisciplinary team and contribute to a patient-centered plan of care, with options for full-time or part-time schedules. A PTA license in Iowa and CPR certification are required, with benefits supporting

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current Physical Therapist Assistant license in the State of Iowa.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Home health experience preferred, but not required.
• Strong communication, organizational, and clinical skills.
• Ability to work independently while maintaining collaboration with the healthcare team.

Responsibilities

• Provide skilled physical therapy treatments in patients' homes under the direction of a licensed Physical Therapist.
• Implement individualized treatment plans to improve mobility, strength, balance, endurance, and safety.
• Assist patients with therapeutic exercises, gait training, transfer training, and functional mobility activities.
• Instruct patients and caregivers on home exercise programs and safety techniques.
• Observe, document, and report patient responses to treatment and progress toward goals.
• Communicate changes in patient condition to the supervising Physical Therapist and interdisciplinary care team.
• Assist with recommendations regarding adaptive equipment and home safety modifications.
• Maintain accurate and timely documentation per agency policies and regulatory requirements.
• Collaborate with nurses, occupational therapists, speech therapists, social workers, and case managers to coordinate care.
• Promote patient independence and quality of life through compassionate, patient-centered care.
