Job summary

Always Best Care of Greater Cleveland is seeking a licensed Physical Therapist to deliver in‑home therapy across Ohio. You will evaluate, treat, and educate patients in their own homes to improve movement, strength, balance, and independence.

Responsibilities include creating individualized plans, supervising PTAs when needed, and ensuring accurate documentation while traveling to patient residences. CPR certification and state licensure required.

Qualifications

• Licensed as a Physical Therapist in Ohio.
• MS/DPT in Physical Therapy or BS with equivalent qualifying experience.
• Current CPR certification.
• Successfully completed the required state certification examination.

Responsibilities

• Provide individualized therapy in patients’ homes and help them work toward safer, stronger movement.
• Evaluate patients and identify physical therapy needs.
• Develop and implement treatment plans based on patient condition and goals.
• Lead therapeutic exercises and hands‑on therapy.
• Monitor progress and adjust treatment as needed.
• Educate patients and families on exercises to continue at home.
• Document evaluations, treatments, and follow‑ups.
• Supervise PTAs when required.
