Job summary

Wellstar Health System, Inc. in Georgia is seeking a dedicated Physical Therapist to provide and supervise primary care for patients in the home health setting.

You will coordinate with RNs, therapists, social workers and family to develop and implement a comprehensive plan of care, ensuring goals are met and quality outcomes. The role requires graduation from an accredited physical therapy program and a current PT license in Georgia, with BLS certification preferred.

Qualifications

• Graduate of an accredited school of physical therapy.
• Minimum 1 year of full-time physical therapy experience; home health experience preferred.
• OASIS knowledge preferred.
• Georgia Driver's License and private vehicle insurance required.

Responsibilities

• Assess and treat patients in the home health setting, coordinating with RNs, therapists and family.
• Develop and manage the plan of care with interdisciplinary teammates.
• Complete OASIS and visit documentation within established timeframes.
• Provide patient and caregiver education to promote self-care and independence.
• Participate in performance improvement activities and stay current with clinical knowledge.
