Job description

We are hiring for a full-time Home Health Physical Therapist/PT to join our team at Willcare in Lake Katrine! This is a field role serving patients in Kingston and surrounding areas.

Compensation for full-time is based on experience, skillset, and case types, with a minimum starting salary averaging around $74,000 per year. Please contact our recruiter for more details!

Responsibilities
  • Evaluate patients' functional status and body systems as required for CMS documentation, and develop therapy plans in consultation with physicians.
  • Provide services within the scope of practice defined by state laws, in accordance with the plan of care, and coordinate with the healthcare team.
  • Observe and report patient responses and changes in condition to supervisors and physicians.
  • Treat patients to relieve pain, restore or develop function, and maintain maximum functioning using evidence-based practices.
  • Instruct and inform healthcare team members, patients, and families about the plan of care, limitations, and progress.
  • Visit patients within the service area and nearby agencies as needed.
  • Complete progress notes for each visit and submit them per policy.
  • Complete patient evaluations and develop PT care plans within state guidelines; report outcomes within 24 hours.
  • Participate in staff meetings and committees as requested.
  • Supervise home health aides and PTAs as permitted by state regulations and policies.
  • Participate in weekend rotations as needed.
  • Prepare discharge summaries and periodic patient condition reports.
  • Provide in-service education upon request.
  • Adhere to LHC Therapy Standards of Practice.
  • Maintain up-to-date clinical knowledge and licensure requirements.
  • Perform other duties as assigned.
Education and Experience

Licensure Requirements:

  • Current Physical Therapy license in the state of practice
  • Current CPR certification
  • Valid driver's license, vehicle insurance, and access to reliable transportation
