Job summary

Enhabit Home Health & Hospice in Georgia is seeking a licensed Physical Therapist (PT) to provide in-home care, evaluate mobility, and develop treatment plans that improve daily living skills and independence for patients.

You will work in a collaborative team, utilize balance and fall prevention programs, and contribute to high-quality home-based therapy. A valid PT license in GA and a driver’s license are required; one year of clinical experience is preferred.

Qualifications

• Must hold an active PT license in the employing state.
• At least 1 year of clinical experience preferred.
• Demonstrate current therapy practice knowledge and skills.
• Valid driver's license and auto liability insurance required.

Responsibilities

• Examine and treat patients with physical impairments using physical modalities.
• Develop and implement treatment plans to improve mobility and daily living skills.
• Utilize specialty programs such as Balance and Fall Prevention, Spine Safety, and joint replacement programs.
