Job summary

Carolina Home Care, part of the Optum family, is seeking a Physical Therapist for Home Health in North Carolina. You will assess patients, develop treatment plans, and deliver interventions in the home setting to improve mobility and pain outcomes.

Requirements include active PT licensure, CPR, and a valid driver's license with reliable transportation. Compensation includes annual target pay, per-visit and hourly pay, and a comprehensive benefits package.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and access to a dependable vehicle or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs; assists the physician in the development of the therapy plan of care.
• Provides services within the scope of practice as defined by state laws and in coordination with the health care team.
• Observes and reports the patient's response to treatment and any changes in condition to the supervising nurse or physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs patient and family regarding the plan of care, functional limitations, and progress toward goals.
• Visits patients per Plan of Treatment and submits progress notes per policy.
• Completes evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and anticipated frequency of care.
• Offers recognition and development opportunities within a supportive environment.
