Job summary

Enhabit Home Health & Hospice is seeking a licensed Physical Therapist to evaluate and treat patients in the home care setting. You will administer skilled therapies to improve mobility, balance, and daily living skills under physician orders, following established plans of care and supervision by the Branch Director.

Specialty programs include Balance and Fall Prevention, Spine Safety, and Total Hip/Knee and Total Shoulder programs.

Qualifications

• Must possess a valid state driver’s license.
• Must possess automobile liability insurance.
• Must wear seatbelts at all times while driving.
• Must have dependable transportation kept in good working condition.
• Must be able to drive an automobile in all types of weather conditions.
• Must be currently licensed in the state of employment.

Responsibilities

• Provide skilled physical therapy to patients under physician orders and plan of care.
• Deliver therapies to improve mobility, independence in daily living, and function.
• Work within specialty programs (Balance and Fall Prevention, Spine Safety, Total Hip and Knee, Total Shoulder) under the Branch Director.
