Position Overview

Aveanna Healthcare has grown tremendously during the past year through a series of exciting acquisitions, and we are currently looking for a Physical Therapist (PT) to join our dynamic home health care team. In this position, you enable patients to recover from illness or injury in the comfort of their own home.

Full-Time visits needed Monday - Friday.

Essential Job Functions

• Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current level of function.
• Administer, evaluate, and interpret tests and measurements to determine functional goals and outcomes.
• Provide written evaluations and plans of care outlining physical therapy needs, goals, progress, and expected outcomes.
• Deliver physical therapy services in accordance with physician orders.
• Maintain plans of care by documenting client performance, modifying goals and treatments as needed, and planning discharges.
• Participate in discharge planning, including educating patients and caregivers on home exercise plans.
• Coordinate services with healthcare team members, physicians, nurses, and others.
• Document according to policies and procedures with minimal supervision.
• Ensure documentation is effective to prevent technical or clinical denials upon payer review.
• Communicate effectively with staff, healthcare team, and families; serve as a resource for clinical staff.
• Supervise other personnel such as PTA and HHA as applicable.
• Complete OASIS assessments where permitted by state regulations.
• Adhere to policies, procedures, and all applicable standards, licensure, and laws.

Requirements

• Degree in Physical Therapy from an accredited program.
• Current, unrestricted state license as a Physical Therapist.
• Valid CPR certification and driver's license.

Preferred Qualifications

• Strong organizational and communication skills.
• Attention to detail and effective time management.
• Ability to build relationships.
• Experience in Home Health Therapy and OASIS documentation.
