Job summary

Carilion Clinic is seeking a Physical Therapist for Home Care, based in Roanoke, with a float team covering surrounding areas. You will assess patients, plan and implement individualized treatment programs, and collaborate with the care team to ensure high-quality outcomes.

Requires Virginia licensure, a PT degree (Bachelor’s minimum) with 1 year of home health experience, BLS within six months, and strong interpersonal skills. Mileage reimbursement and sign-on bonus eligibility offered.

Qualifications

• Graduate of approved school of Physical Therapy.
• Licensed to practice in the Commonwealth of Virginia.
• BLS certification within six months of employment.

Responsibilities

• Provides patient care based on physician referral and age-appropriate assessment.
• Coordinates patient care with therapists, clinicians, and physicians.
• Documents in the medical record per regulatory requirements.
• Supervises PTAs and collaborates with therapy staff and students.
• Participates in in-services, performance improvement, and committee work.
• Delivers compassionate, respectful patient care and adheres to company values.
