Qualifications

• Degree from an accredited Physical Therapy program.
• Valid Maine PT license.
• At least two years of PT experience; home health preferred.
• Strong verbal and written communication; organized.
• Current BLS CPR certification; valid driver's license and vehicle.

Responsibilities

• Deliver PT services per physician’s plan of care and state Act.
• Assess therapeutic status and home environment for rehab potential.
• Measure strength, mobility, gait, ROM; identify rehab opportunities.
• Administer PT treatments and instruct use of devices.
• Report treatment reactions or condition changes to physician.
• Coordinate with healthcare team and family caregivers.
• Educate patient and caregivers on therapy goals and home exercises.
• Identify additional home health services needed and refer as required.
• Contribute to discharge planning and case conferences.
• Prepare progress summaries and documentation.
