Job summary

Sunrise Home Health Services is seeking a compassionate Physical Therapist to join our Scottsdale home health team. This role offers flexible full-time or part-time scheduling, clinical autonomy, and a supportive leadership structure to deliver one-on-one patient care.

You will evaluate patients in their homes, design plans of care, and provide interventions to improve mobility, balance, and independence while coordinating with the care team and ensuring compliant documentation.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current and unrestricted Arizona Physical Therapist license.
• CPR Certification (or ability to obtain).
• Valid driver's license and reliable transportation.
• Strong clinical judgment, communication, and organizational skills.
• Ability to work independently while collaborating effectively with an interdisciplinary team.
• Home health experience is preferred but not required.
• New graduates are encouraged to apply!

Responsibilities

• Perform comprehensive physical therapy evaluations in patients' homes.
• Develop individualized plans of care based on patient needs and medical condition.
• Provide skilled PT interventions to improve mobility, strength, balance, and independence.
• Establish measurable goals and monitor patient progress.
• Modify treatment plans as clinically appropriate.
• Educate patients and caregivers on home exercise programs and safety.
• Assess need for assistive devices and home safety modifications.
• Coordinate care with PTAs when applicable.
• Communicate progress with physicians and care team.
• Ensure timely clinical documentation per Medicare and agency rules.
• Collaborate with nurses, therapists, and physicians to deliver patient-centered care.
