Trio Home Health in Denton, TX is seeking a dedicated Physical Therapist (full-time, part-time or PRN) to provide compassionate care to patients in home health settings. You will assess, plan, and implement individualized therapy programs across age groups, coordinating with the care team to optimize outcomes.

Qualifications

• Bachelor of Science in Physical Therapy from an APTA-approved program.
• Current state license or PTLC multistate license as applicable.
• At least 1 year of PT experience in acute care, rehab, or home health preferred.
• Proficiency in therapy skills, strong interpersonal skills, CPR certification, DME knowledge.
• Reliable transportation, valid driver's license, and auto liability insurance.

Responsibilities

• Develop and implement individualized physical therapy treatment plans.
• Evaluate and document progress; adjust treatments as needed.
• Coordinate care with other healthcare professionals.
• Educate patients and families on plans, home exercises, and prevention.
• Maintain accurate patient records per standards.
• Utilize durable medical equipment and provide training.
• Ensure safety and comfort during therapy sessions.
• Pursue ongoing professional development.
