Job summary

Pinnacle Home Care in Florida is hiring full-time or PRN Physical Therapists to deliver in-home care across Lehigh Acres, Ft. Myers, Estero and nearby areas.

You will assess patients, develop personalized treatment plans, supervise assistants, and collaborate with physicians, while maintaining accurate records and promoting high-quality, patient-centered outcomes.

Qualifications

• Must have a current PT license (state-specific).
• Graduate of an accredited physical therapy program.
• Valid BLS certification.
• Minimum one year of PT experience (home health preferred).
• Excellent communication, organizational, and time management skills.
• Must have a valid driver’s license and access to a reliable and insured vehicle.
• Ability to visit homes that may not be wheelchair accessible.
• Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
• Proficiency with electronic medical records is a plus.

Responsibilities

• Perform initial and ongoing patient assessments, including OASIS evaluation and documentation.
• Develop and implement tailored treatment plans in consultation with physicians and clinicians, adjusting as needed.
• Collaborate with the interdisciplinary care team to optimize patient outcomes.
• Provide education and support to patients and families/caregivers on therapy programs.
• Maintain accurate clinical records and report any changes in the patient’s condition to the physician.
• Supervise Physical Therapy Assistants in accordance with agency policies and state regulations.
• Participate in in-services, case conferences, and meetings to promote coordinated care with healthcare team members.
• Identify patient and family/caregiver needs for additional services and make appropriate referrals.
• Confirm daily visit schedules to ensure seamless coordination with other staff.
• Communicate patient progress and changes with physicians and care teams.Document all visits and progress notes on the same day of service.
