Job summary

Concierge Home Care is seeking a Home Health Physical Therapist (PT) to deliver 1:1 patient care in the home across St Augustine and surrounding areas. You will enjoy a flexible schedule, autonomy, and the opportunity to increase income based on productivity.

The role supports ongoing clinical mentoring, onboarding, and optional CEUs, with a compensation structure targeting approximately $95,000+ annually.

Qualifications

• Active Florida PT license in good standing.
• Reliable vehicle and valid driver’s license.
• Willingness to travel within St Augustine/St Johns County.

Responsibilities

• Evaluate patients and develop individualized home treatment plans.
• Provide therapeutic exercises and mobility training.
• Educate patients and caregivers on safety and recovery.
• Collaborate with physicians and interdisciplinary teams.
• Supervise PTAs as needed.
• Complete documentation using streamlined EMR.
