Job summary

Homeside Rehab is seeking a compassionate Physical Therapist to join our home health team in Irvington, NJ. You will deliver individualized, evidence-based therapy to patients in their homes, with flexible scheduling and a supportive team environment.

Responsibilities include evaluating patients, creating treatment plans, providing PT interventions to improve function, and maintaining compliant documentation. A NJ PT license and valid driver's license are required.

Qualifications

• Active NJ Physical Therapy License (PTA) — required
• Basic computer proficiency for electronic documentation
• Strong clinical assessment and interpersonal communication skills
• Ability to work independently in a home health environment
• Valid NJ driver's license and reliable transportation for home visits
• Minimum 1 year of experience in Home Health, Outpatient, or SNF settings

Responsibilities

• Evaluate patients and develop individualized treatment plans based on clinical assessment and patient goals
• Provide evidence-based PT interventions to improve motor skills, functional independence, and developmental outcomes
• Maintain accurate, timely documentation of all patient care in accordance with regulatory requirements
• Collaborate with the clinical team to ensure coordinated and effective treatment
• Communicate clearly with office staff, patients, and caregivers regarding progress and care plans
