Qualifications

• Home Health experience preferred, not required.

Responsibilities

• Provides PT services to patients in home settings under physician plan of care.
• Participates in case conferences to ensure continuity and quality of care.
• Demonstrates sound judgment and technical skills per agency protocols.
• Maintains compliance with ethics and agency standards.
• Performs safety, infection control, and workload tasks as assigned.
