Job summary

Pride Health is seeking a licensed Physical Therapist to support a client’s home health program in Annapolis, MD. This local contract position runs Monday through Friday, 8:00 AM to 4:30 PM, with a 14-week contract length and pay range of $51.00–$56.00 per hour.

Qualifications

• Active Maryland Physical Therapist (PT) License.
• Graduate of an accredited Physical Therapy program.
• Home Health experience required.
• Ability to independently evaluate and treat patients in a home-based setting.
• Current driver's license, reliable transportation, and automobile insurance.
• Strong clinical assessment and patient education skills.

Responsibilities

• Perform home-based PT evaluations and assessments.
• Develop and update individualized treatment plans per physician orders.
• Treat medical-surgical, orthopedic, neurological, and mobility-impaired patients at home.
• Conduct recertifications, reassessments, and discharge evaluations as needed.
• Monitor progress and adjust plans to optimize outcomes.
• Educate patients and caregivers on exercises, mobility, fall prevention, and home programs.
• Collaborate with physicians, nurses, and therapists.
• Complete accurate clinical documentation per regulations.
• Meet productivity standards while delivering high-quality care.
