Qualifications

• Minimum two years of physical therapy experience.

• National Provider Identifier required at hire.
• Basic Life Support required at hire (AHA).

Responsibilities

• Evaluates patients and plans care for home health visits.
• Provides PT services as prescribed and evaluates response.
• Maintains infection control and safe environment.
• Coordinates plan of care with physicians, patients, and caregivers.
• Supervises Home Care Aides and PTAs and conducts supervisory visits.
• Performs case management and documents per standards.
