Responsibilities

• Adheres to policies and plan of care under supervision.
• Implements treatment to improve or minimize disability and restore function.
• Communicates progress and plans with physicians and care team.
• Completes and submits OASIS assessments and related documentation.
• Travels to patient homes and collaborates with family and care staff.
