Responsibilities

The Physical Therapist in Home Health is responsible for the assessment and evaluation of patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment, the Physical Therapist develops a treatment plan, performs interventions to improve patient well-being, and evaluates progress. They assess the patient's functional status, body systems as required for CMS documentation, and physical therapy needs. They also consult with the physician for developing the therapy plan of care.

  • Provide services within the scope of practice as defined by state laws, in accordance with the plan of care, and in coordination with the healthcare team.
  • Observe and report patient responses and condition changes to the nurse supervisor and/or physician.
  • Treat patients to relieve pain, restore and develop function, and maintain maximum functioning using accepted standards and evidence-based techniques.
  • Instruct and inform team members, patients, and families about the plan of care, limitations, and progress.
  • Visit patients within the service area and nearby agencies as needed.
  • Complete progress notes for each visit and submit them per policy.
  • Complete evaluations and develop the PT plan of care within state guidelines, reporting outcomes within 24 hours.
  • Participate in staff conferences and committees as requested.
  • Supervise home health aides and PTAs as allowed by state regulation and policy.
  • Participate in a weekend rotation as needed.
  • Prepare discharge summaries and periodic patient condition reports.
  • Provide in-service education as needed.
  • Adhere to LHC Therapy Standards of Practice.
  • Maintain and update clinical knowledge and licensure requirements.
  • Perform other duties as assigned.
Education and Experience

Licensure Requirements

  • Current Physical Therapy licensure in Illinois
  • Current CPR certification
  • Valid driver's license, vehicle insurance, and dependable transportation
