Job description

Seeking Home Health Physical Therapist (PT) to work in the Dallas area (downtown, south and southwest). Pay rate depending upon experience. Must have reliable transportation and willing to provide therapy services in patient's home.

Home Health PT Requirements :

  • Physical Therapy degree
  • Licensed in the State of Texas to practice as a Physical Therapist
  • Good communication skills (written and verbal)

Responsibilities include, but not limited to :

  • Conduct evaluations and assessments to determine the appropriate treatment plan for each patient in their home environment.
  • Develop treatment plans by identifying a problem list, including long and short-term goals and methods to achieve identified goals.
  • Participate in the care planning process, document the progress toward identified goals and communicating the patient's status and needs to the patient, the patient's family, and staff who has a need to know.
  • Develop an appropriate home program to maintain and enhance the performance of the patient in his / her own environment.
  • Participate in the plan for discharge which may include recommendation for adaptive devices; and training for patient, family and appropriate staff on how to use the devices.
  • As required, supervise Physical Therapy Assistants of patient's treatment plan.
  • Knowledge of software used to develop and upload reports.
