Qualifications

• Active California therapy license required.
• Minimum 1 year of licensed experience.
• Valid driver's license, reliable transit and auto insurance.
• CPR certification and professional liability insurance.
• Recent physical exam (within 2 years) and TB clearance.
• Active EIN and business license/city tax certificate.
