We are hiring for a full-time Home Health Physical Therapist. This is a field role serving patients in Newton, Chestnut Hill, Brookline, and Brighton.

Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health is responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment, the Physical Therapist develops a treatment plan, performs interventions to improve and enhance patient well-being, and evaluates progress.

• Evaluates the patient's functional status and needs. Assists the physician in developing the therapy plan of care.
• Observes and reports the patient's response to treatment and any condition changes to the supervising nurse and/or physician.
• Treats patients to relieve pain, restore function, and maintain maximum functioning according to standards of practice.
• Instructs and informs the healthcare team, patient, and family/caregivers about the plan of care, limitations, and progress toward goals.

Education and Experience

Licensure Requirements:

• Current Physical Therapy licensure in MA
• Current CPR certification
• Valid driver’s license, vehicle insurance, and access to dependable transportation
