Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health assesses and evaluates patient needs related to functional status, mobility, endurance, pain, and other medical conditions. Based on this, the therapist develops treatment plans, performs interventions to improve patient well-being, and monitors progress.

• Evaluate the patient's functional status and therapy needs, assisting in developing the plan of care with the physician.
• Observe and report patient responses and condition changes to the supervising nurse or physician.
• Treat patients to relieve pain, restore or develop function, and maintain maximum functioning according to accepted standards.
• Educate and inform the healthcare team, patients, and families about the care plan, limitations, and progress.

Education and Experience

Licensure Requirements

• Current Physical Therapy license in the state of practice
• Current CPR certification
• Valid driver's license, vehicle insurance, and access to a dependable vehicle
