Job summary

Elliot Health System’s Visiting Nurse Association in Manchester, NH is seeking a licensed Physical Therapist for home care. This full-time on-site role delivers in-home PT services across southern New Hampshire with day shifts and weekend rotation.

You will assess clients, design and adjust therapy plans, document care on a laptop, and collaborate with the care team to support patients recovering from surgery or managing disabilities. A NH PT license, BLS/CPR, and driving is required.

Qualifications

• Degree from an American Physical Therapy Association (APTA) accredited program required.
• Licensed as a Physical Therapist by the NH Office of Licenses Allied Health Professionals required.
• BLS/CPR certification required.

Responsibilities

• Plans, develops, and implements physical therapy care plans for clients across ages and complexity.
• Regularly re-evaluates and updates care plans and physicians' orders.
• Communicates information to patients, physicians, peers, managers, and other agencies; documents care.
• Orients and supervises PTAs according to NH guidelines and VNA policy.
