Job summary

Caring Hand Home Health Solutions, Inc. is seeking a Physical Therapist Assistant to work alongside a licensed Physical Therapist in patients' homes. The role focuses on helping patients recover mobility and return to daily activities with hands-on therapy and guided exercises.

Per diem position with a visit-based rate, requiring an Associate's degree in PTA, active CA PTA license, and CPR (AHA). Candidates must be able to travel to patient homes and maintain clear communication with the care

Qualifications

• Associate's degree from an APTA-accredited Physical Therapy Assistant program.
• Active PTA license from the Physical Therapy Board of California.
• Current CPR certification with AHA.
• Strong communication and organizational skills.
• Valid driver's license and willingness to travel to patient homes.

Responsibilities

• Deliver hands-on physical therapy under the direction of a licensed Physical Therapist, following each patient's care plan.
• Guide patients through therapeutic exercises, gait and transfer training, and the use of assistive devices.
• Apply physical modalities including heat, cold, ultrasound, electrical stimulation, and massage to relieve pain and restore function.
• Monitor patient progress and communicate changes to the Physical Therapist and care team.
• Document care accurately and on time, and participate in team meetings and ongoing training.
• Perform other duties as required for patient services
