Job summary

Addus Family Company seeks a compassionate Physical Therapist Assistant to deliver high-quality, in-home rehabilitation services under the direction of a licensed Physical Therapist. You’ll implement individualized treatment plans to improve mobility, strength, balance, and safety, helping seniors and individuals with disabilities remain independent at home.

You will provide hands-on care, monitor and document progress, educate clients and caregivers on exercises and safe mobility techniques,

Qualifications

• Licensed PTA required (state-specific).
• Experience delivering in-home rehabilitation.
• Ability to educate clients and caregivers on exercises.

Responsibilities

• Deliver high-quality, in-home rehabilitation under supervision of a licensed Physical Therapist.
• Implement individualized treatment plans to improve mobility, strength, balance, and safety.
• Provide hands-on care, monitor and document progress, educate clients and caregivers.
• Collaborate with the interdisciplinary care team to promote independence at home.
