Job summary

Remedy Home Health Care is actively hiring a Physical Therapist Assistant (PTA) to join our home health team in South Orange County. This per diem role offers flexible scheduling including part-time or full-time options, delivering one-on-one rehabilitative care to seniors in their homes.

You will provide skilled therapy under supervision of a licensed PT, focusing on rehabilitation, fall prevention, and functional mobility, with documentation in WellSky Kinnser EMR.

Qualifications

• Active California PTA license required.
• Current CPR certification (AHA) required.
• Minimum 1 year of clinical experience (home health preferred).
• Valid driver’s license, auto insurance, and reliable transportation.

Responsibilities

• Provide in-home physical therapy services following the Plan of Care.
• Assist patients with therapeutic exercises and mobility training.
• Support senior care patients in improving strength, balance, and independence.
• Monitor and report patient progress to supervising PT.
• Educate patients and caregivers on home exercise and safety.

Your Role as a Home Health PTA (Physical Therapist Assistant)

As a Physical Therapist Assistant in home health, you will deliver skilled therapy services under the supervision of a licensed Physical Therapist, focusing on rehabilitation, fall prevention, and functional mobility.

Key Responsibilities:

• Provide in-home physical therapy services following the established Plan of Care


• Assist patients with therapeutic exercises, mobility training, and rehabilitation programs


• Support senior care patients in improving strength, balance, and independence


• Monitor, document, and report patient progress to the supervising Physical Therapist


• Educate patients and caregivers on home exercise programs, safety, and fall prevention


• Collaborate with physicians, RNs, and the interdisciplinary care team


• Complete documentation using WellSky Kinnser EMR

Qualifications:

• Active California Physical Therapist Assistant (PTA) license


• Current CPR certification (AHA required)


• Minimum 1 year of clinical experience (home health experience preferred)


• Strong interest in home health, rehab, and senior care


• Excellent communication and organizational skills


• Valid driver’s license, auto insurance, and reliable transportation
