Bayhealth by BAYADA Home Health Care is currently seeking an experienced Physical Therapist, PT, for a full time opportunity performing home health visits to adult and geriatric clients on a per visit basis in territories throughout the Southern Sussex county area.

Available PT Shifts:

• Full Time: 25 visits per week M-F
• Per Diem: 5-10 visits per week M-F

• Assessment and Treatment: Conduct thorough evaluations, develop personalized treatment plans, and implement evidence‑based interventions to optimize patient outcomes.
• Collaboration: Work closely with other healthcare professionals, patients, and their families to provide comprehensive care and achieve the best possible results.
• Documentation: Accurately document patient progress, treatments, and outcomes in accordance with BAYADA policies and regulatory requirements.

Qualifications:

• A current and valid Physical Therapist license in the state of Delaware.
