Job summary

Jethro Health System is seeking a Home Health Physical Therapist to plan, organize, and direct home care services for patients in the North Shore and Greater Boston area.

You will assess patients, develop individualized plans, coordinate with PTA staff, and communicate with physicians while ensuring HIPAA compliance and accurate OASIS documentation.

Qualifications

• Must hold a degree from an accredited Physical Therapy program.
• Massachusetts Physical Therapist license is required.
• Active BLS certification is required.
• 2+ years of PT experience, especially in public health or home care.
• OASIS experience is preferred.

Responsibilities

• Coordinate care for assigned patient caseload, delegating to and supervising PTA staff if applicable.
• Complete and document an initial patient assessment to determine home care needs.
• Develop a patient-specific plan of care with goals involving patient and family.
• Initiate and revise the plan of care as patient status changes; start preventive and rehabilitative therapy.
• Communicate with the physician regarding patient needs and changes in condition.
• Maintain patient confidentiality per HIPAA and agency policies.
• Demonstrate proficiency in OASIS completion and timely documentation.

Job Description:

What You’ll Do: The physical therapist plans, organizes, collaborates, and directs home care services. The professional therapist builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities in accordance with home care regulations and payer guidelines. As a home health physical therapist you will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to clients, allowing them to receive the medical care required to maintain their independence.

Responsibilities:

• Coordinate care for assigned patient caseload, including but not limited to: delegating to and supervising of PTA staff (if applicable), processing patient condition change visits, and completing OASIS time points.
• Complete and document an initial assessment of the patient and family to determine home care needs.
• Provide a complete physical assessment and history of current and previous illness(es) and develop a patient‑specific plan of care to establish goals incorporating therapeutic, preventive, and rehabilitative actions by including the patient and the family in the planning process.
• Initiate the plan of care and make necessary revisions as patient status and needs change. Initiate appropriate preventive and rehabilitative therapy procedures.
• Communicate with the physician regarding patient needs and report any changes in patient condition; obtain/receive physician’s orders as required.
• Practice confidentiality principles set by the agency and federal HIPAA guidelines.
• Demonstrate proficiency in OASIS completion; complete documentation in accordance with agency timeliness policies.

Requirements:

• Degree from an accredited program in Physical Therapy; DPT preferred.
• Current valid Massachusetts Physical Therapist license;
• Active American Health Association BLS certification;
• 2+ years of physical therapy experience, at least one of which is in the area of public health or home care;
• OASIS experience is preferred.
