BAYADA Home Health Care in Somerville, New Jersey, seeks a Physical Therapist to join our Central NJ Senior Living team. This full-time role involves hybrid work, visiting clients in Somerset/Middlesex counties and performing comprehensive PT duties with a focus on 1:1 care.

Responsibilities include home visits, patient assessment, gait training, wound care, and education for clients and families. Sign-on bonus of $20,000 is offered for full-time PTs in the region.

Qualifications

• A current NJ Physical Therapist license.
• Minimum of one year of recent PT experience.
• Graduation from an APTA-approved PT program (or equivalent).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, gait, balance, strength, endurance, safety).
• Implement wound care and protective interventions.
• Develop and revise individualized care plans with manual therapy, gait training, balance, and other PT interventions.
• Collaborate with nursing in multidisciplinary care plans as appropriate.
• Educate clients and caregivers on rehabilitative care and activities.
• Accurately document observations, interventions, and evaluations in an EMR.

BAYADA Home Health Care is seeking a Physical Therapist (PT) for a full time position with our new Central NJ Senior Living team - a Medicare Certified home health office.

This office provides best-in-class care to adult and geriatric clients living in communities that include assisted living, independent living, senior congregate living, 55+ communities, and intermediate care facilities for clients with intellectual disabilities throughout Somerset and Middlesex County.

*This is a hybrid position that may also include seeing clients in the traditional home health community setting.

Responsibilities:

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
• Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence-based dressings and topical agents.
• Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
• Continually assess and revise the Physical Therapy - PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
• Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.

Qualifications:

• A current NJ Physical Therapist - PT license.
• A minimum of one year of recent work experience as a Physical Therapist - PT.
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.

Additional Information:
Base Pay, depending upon qualifications and subject to negotiation:
$65.00 - $75.00 per point
