BAYADA Home Health Care, Inc. is seeking a Home Health Physical Therapist for a full-time role in Raritan, New Jersey, serving adult and geriatric clients in senior living communities and home health settings.

The position is hybrid, with potential visits in traditional home health environments. Responsibilities include performing mobility assessments, creating care plans, educating families, and documenting care using EMR.

Formación

• A current NJ Physical Therapist license.
• Minimum of one year of PT work experience.
• Graduation from a program approved by the APTA or AMA commission.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.

Responsabilidades

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measures of mobility, gait, strength, balance, and safety.
• Implement wound care interventions with evidence-based dressings and topical agents.
• Develop and update individualized care plans with therapeutic exercises and gait training.
• Assess and revise the PT care plan and collaborate with nursing on the multidisciplinary team.
• Educate clients and families in rehabilitative care to promote health and independence.
• Document observations and interventions using a state-of-the-art tablet.

BAYADA Home Health Care is seeking a Physical Therapist (PT) for a full time position with our new Central NJ Senior Living team - a Medicare Certified home health office.

This office provides best-in-class care to adult and geriatric clients living in communities that include assisted living, independent living, senior congregate living, 55+ communities, and intermediate care facilities for clients with intellectual disabilities throughout Somerset and Middlesex County.

*This is a hybrid position that may also include seeing clients in the traditional home health community setting.

Responsibilities:

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
• Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence-based dressings and topical agents.
• Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
• Continually assess and revise the Physical Therapy - PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
• Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the‑art touch pad tablet.

Qualifications:

• A current NJ Physical Therapist - PT license.
• A minimum of one year of recent work experience as a Physical Therapist - PT.
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.

Additional Information:
Base Pay, depending upon qualifications and subject to negotiation:
$65.00 - $75.00 per point
