Job Description & Requirements

Advocate Aurora Health is seeking a Home Health Physical Therapist for a position in Glenwood, Illinois.

Responsibilities
  • Ensure and improve quality of care by utilizing appropriate resources, adhering to evidence-based standards, and participating in quality initiatives.
  • Collaborate with the care team for effective care coordination and participate in organizational activities.
  • Demonstrate leadership, make sound clinical decisions, and evaluate changes in patient conditions.
  • Build and maintain strong patient relationships, providing education and involving families in care planning.
  • Enhance productivity and monitor resource utilization for optimal outcomes.
  • Meet patient education needs with a systematic approach, assessing learning readiness and providing appropriate resources.
Qualifications
  • Illinois or Wisconsin Physical Therapy License and Basic Life Support (BLS) certification.
  • Doctorate in Physical Therapy and at least 1 year of clinical experience or equivalent.
  • Proficiency in computer use, good communication skills, and experience with home therapy equipment.
Physical Requirements & Working Conditions
