Job summary

CenterWell Home Health is seeking a licensed Physical Therapist to deliver in-home therapy, helping patients regain strength and independence. You will assess, plan and implement rehabilitation programs, coordinate with the care team, and document progress and outcomes.

You will work onsite in patients' homes with a 40-hour schedule, and you must have valid licensure, CPR certification, and a collaborative approach to care. Sign-on bonus and relocation options may apply.

Qualifications

• Current and unrestricted Physical Therapy license is required.
• Current CPR certification is required.
• Experience collaborating with a healthcare team.

Responsibilities

• Plan and administer prescribed physical therapy treatments in patients' homes to improve function.
• Assess patients' physical strengths to guide treatment goals and support physician evaluation.
• Develop and implement rehabilitation programs aligned with the physician's plan.
• Provide clear instructions to PTAs and Home Health Aides and oversee documentation.
• Document observations, interventions and evaluations promptly and thoroughly.
• Report progress to the physician, Clinical Manager, care team, patient and family; submit plans and discharge summaries.
