Job summary

CenterWell Home Health is seeking a Physical Therapist to deliver home-based care, plan and administer prescribed therapy to restore function and independence for patients in their homes. Reporting to the Clinical Manager, you will assess patients, coordinate care with a multidisciplinary team, and document progress while ensuring compliance with all regulations.

This role includes a $10,000 sign-on bonus or relocation assistance, with benefits starting on day one and a company vehicle program

Qualifications

• Active unrestricted Physical Therapy license.
• Current CPR certification.
• Experience working with a healthcare team.

Responsibilities

• Plan and administer prescribed physical therapy in patients' homes to improve function.
• Assess and document patients' strengths to guide goals and physician evaluations.
• Develop and implement rehab programs aligned with the Plan of Treatment.
• Provide clear instructions to PTAs and Home Health Aides and oversee documentation.
• Document observations, interventions, and progress promptly.
• Report progress to physician, Clinical Manager, care team, patient, and family; submit evaluations and discharge summaries.
