Job summary

Ascension at Home together with Compassus is seeking a Home Health Physical Therapist to deliver one-on-one care in patients' homes, helping them regain strength and independence. You will assess, treat, and document progress, coordinating with families and care teams to support safe, functional daily living.

Qualifications

• Bachelor of Science in Physical Therapy is required.
• Current State License as a Physical Therapist.
• Valid Driver’s License.
• Current CPR, negative TB screen and Hepatitis consent/declination.
• Two years’ experience as a Physical Therapist in an acute care or rehabilitation setting.
• Good interpersonal skills.
• Knowledge of durable medical equipment.

Responsibilities

• Provide in-home physical therapy services one-on-one.
• Assess patient goals and develop treatment plans.
• Improve independence in daily living activities through therapy.
• Educate patients and caregivers on exercises and home programs.

Minimum Qualifications:

• Bachelor of Science in Physical Therapy
