Job summary

Ascension at Home together with Compassus is hiring a Home Health Physical Therapist to deliver one-on-one therapy in patients' homes across our service area. You will assess mobility, design personalized plans, and guide patients toward greater independence with strength and balance training.

Qualifications

• Bachelor of Science in Physical Therapy is required.
• Current State License as a Physical Therapist is required.
• Valid Driver’s License is required.
• Current CPR, negative TB screen and Hepatitis consent/declination are required.
• Two years’ experience as a Physical Therapist in an acute care or rehabilitation setting; home health experience strongly preferred.

Responsibilities

• Provide home-based physical therapy to patients one-on-one in their homes.
• Assess, plan, and implement therapy to improve mobility, strength, and independence.
• Document progress and communicate with care teams to optimize patient outcomes.

Work one-on-one with patients when and where they need the most help – in their home! As a Home Health Physical Therapist, you have the unique opportunity to make a huge difference in your patients’ lives and see firsthand how your work impacts their ability to live in their home while gaining the strength and independence they need to participate in their desired daily living activities, such as going to church or working in the garden.

Minimum Qualifications

• Bachelor of Science in Physical Therapy
• Current State License as a Physical Therapist
