Job summary

The Roads Home Health Care in South Florida is seeking a licensed Home Health Physical Therapist to provide skilled care in patients' homes. You will evaluate patients, design individualized treatment plans, and help regain strength and mobility after illness, injury, or surgery.

Both full-time and per diem opportunities are available. Key duties include in-home assessments, developing plans, delivering modalities, educating patients and caregivers, coordinating with physicians, and supervising

Qualifications

• Current Florida Physical Therapist license or eligible.
• Graduate of an accredited physical therapy program.
• Minimum of two years of physical therapy work experience.
• Current CPR/BCLS certification and TB clearance

Responsibilities

• Perform in-home patient assessments and develop individualized PT treatment plans.
• Obtain and communicate physician orders for certification and recertification of home PT services.
• Deliver physical therapy modalities and hands-on treatment tailored to each patient.
• Design home exercise programs aligned with patient goals and functional status.
• Assess and recommend durable medical equipment and adaptive equipment needs.
• Evaluate and manage patient pain through PT interventions.
• Educate patients and family caregivers on exercises and home safety.
• Identify home safety and mobility risks and recommend interventions.
• Provide supervision to physical therapist assistants.
• Complete timely, accurate documentation and coordinate care with physicians and the interdisciplinary team

About the Role

The Roads Home Health Care is seeking a licensed Home Health Physical Therapist to provide skilled physical therapy and rehabilitative patient care to patients in their homes throughout South Florida. In this role, you'll evaluate patients, design individualized treatment plans, and help people regain strength, mobility, and independence after illness, injury, or surgery. Both full-time and per diem opportunities are available for the right candidate.

• Perform comprehensive in-home patient assessments and develop individualized physical therapy treatment plans
• Obtain and communicate physician orders for certification and recertification of home physical therapy services
• Deliver physical therapy modalities and hands-on treatment appropriate to each patient's condition, needs, and age
• Design home exercise programs tailored to each patient's goals and functional status
• Assess patients for durable medical equipment and adaptive equipment needs
• Evaluate and manage patient pain through appropriate physical therapy interventions
• Educate patients and family caregivers on techniques, exercises, and home safety
• Identify safety and mobility risks in the home environment and recommend interventions
• Provide clinical supervision and mentorship to physical therapist assistants
• Complete timely, accurate documentation and coordinate care with physicians and the interdisciplinary team

Job Requirements

• Current, valid Florida Physical Therapist license (or eligibility per state and CAPTE-accredited program requirements)
• Graduate of an accredited physical therapy program
• Minimum of two years of physical therapy work experience
• Current CPR/BCLS certification
• Current health certificate, physical exam, and TB clearance
• Valid driver's license, reliable vehicle, and current auto insurance; ability to pass a criminal background check

Qualifications

• Strong clinical judgment with the ability to synthesize assessment findings into an effective care plan
• Excellent verbal and written communication skills for patient education and interdisciplinary coordination
• Ability to work independently while managing a home visit schedule efficiently
• Compassionate, patient-centered approach to home health and rehabilitative care
• Strong organizational skills and attention to documentation detail
• Prior home health experience preferred
• Bilingual candidates a plus
