Job description

Parx Home Health Care is seeking a talented Per Diem Physical Therapist to join our team. The Physical Therapist (PRN) will provide high-quality, coordinated care to patients in their homes and communities. The ideal candidate will have a strong background in home health care, knowledge of relevant regulations, and a passion for patient well-being.

What you'll be doing:

1. Conduct comprehensive physical therapy evaluations and create personalized treatment plans.
2. Teach patients and families exercises and activities to promote recovery and mobility.
3. Document patient progress and communicate updates to healthcare team members.
4. Coordinate care with other medical professionals to ensure effective treatment.
5. Follow policies and safety protocols, including infection control.
6. Educate on injury prevention and self-care techniques.
7. Maintain accurate records in compliance with regulations.

What we expect from you:

1. Graduate from an accredited physical therapy program with a valid license in Florida.
2. At least one year of experience, preferably in home health care.
3. Ability to develop effective treatment plans.
4. Strong communication skills.
5. Ability to work independently and as part of a team.
6. Ability to interpret physician orders and implement treatments.
