Angelcare Home Health Services is seeking a Physical Therapist contracted or employed to provide therapy per physician plan of care and to align with state guidelines. The role involves delivering services in the home health setting and collaborating with the broader care team.

Ideal candidates hold a California PT license, possess a degree in physical therapy, CPR certification, and at least two years of related experience; community/home health experience is preferred.

Qualifications

• Must hold a degree in physical therapy from an accredited program.
• Licensed to practice in California as a Physical Therapist.
• Two years of PT experience; home health/community preferred.
• Current CPR certification.

Responsabilités

• Provides physical therapy services per physician's plan of care and state requirements.
• Initiates therapy program and educates staff and family on goals and procedures.
• Trains patient in the use of prosthetic devices as needed.
• Participates in discharge planning for the patient.
• Prepares and submits clinical and progress summaries on goal attainment.
• Instructs, supervises and evaluates home health aide care every two weeks when therapy is the only skilled service.

The physical therapist contracted or employed through the Organization is responsible for providing physical therapy services and for adherence to all conditions outlined in the Personnel Services Agreement.

Essential Job Functions/Responsibilities

• Provides physical therapy services to patients according to a written physician's plan of care and as defined in the state Physical Therapy Practice Act.
• Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences.
• May train patient in the use of prosthetic device.
• Participates in discharge planning for patient.
• Prepares and submits clinical and progress summaries based on the attainment of goals.
• Prepares and submits a clinical progress summary based on the attainment of goals as directed by Organization policy.
• When therapy is the only skilled service, instructs, supervises and evaluates home health aide care every two (2) weeks.

Qualifications

• Possesses a degree from a baccalaureate or master's program in physical therapy approved by an accredited organization.
• Licensed to practice as physical therapist by the Physical Therapy Board of California.
• Two years of appropriate experience as a physical therapist. Community/home health experience is preferred.
• Possesses and maintains current CPR certification.
• Demonstrates good verbal and written communication and organization skills.
