Job summary

Active Health Services in California is seeking a Physical Therapist (PT) to provide physical therapy services per physician's plan of care in the North County area.

The PT will assess and treat patients, educate families, supervise PTAs, and participate in discharge planning with a focus on home health settings. A CA PT license and CPR certification are required, with 2+ years of PT experience preferred; valid driver with insured vehicle is necessary.

Qualifications

• CA Physical Therapy License is required.
• 2+ years of physical therapy experience, home health preferred.
• Must demonstrate good verbal and written communication and organization skills.
• Must possess CPR certification and be able to drive with a valid, insured vehicle.

Responsibilities

• Assess and evaluate therapeutic/rehabilitative status and plan of care.

JOB DESCRIPTION

(North County Area) Active Health Services is seeking a Physical Therapist (PT). The PT is responsible for providing physical therapy services to patients according to a written physician's plan of care.

DUTIES AND RESPONSIBILITIES

• Assess and evaluate therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care
• Evaluate home environment
• Assess for muscle strength, mobility, gait, ROM - potential for rehab
• Direct physical therapy treatment
• Instruct patients and families/caregivers in the use and care of therapeutic appliances
• Determine priority needs for physical therapy
• Report to physician patient's reaction to treatment or changes in condition
• Initiate physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient
• Instruct them as to the goals of the physical therapy program for the patient by participating in case conferences
• Train patient in the use of prosthetic device
• Identify patient and family/caregiver needs for other home health services
• Prepare and submit clinical and progress summaries based on the attainment of goals
• Participate in discharge planning for patient
• Prepare and submit a clinical progress summary based on the attainment of goals
• Provide physical therapy consultation to home families/caregivers
• Provide in-service education programs for nursing organization personnel
• Participate in peer consultation process
• Supervise Physical Therapy Assistants according to organization policy and state regulations

JOB REQUIREMENTS

• CA Physical Therapy License
• Two (2)+ Years of physical therapy experience, home health experience preferred
• Demonstrate good verbal and written communication and organization skills
• Possess and maintain current CPR certification
• Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order
