Job summary

PARS Therapy is seeking a compassionate and skilled Physical Therapist (PT) to join our home health team in Port Arthur, TX. This role involves delivering patient-centered, evidence-based care directly in patients' homes, with a focus on reducing pain and improving mobility.

You will evaluate, treat, and educate patients and caregivers, document progress, and collaborate with an interdisciplinary team to ensure optimal outcomes.

Qualifications

• Active PT license in TX required.
• Graduate degree or higher in Physical Therapy from an accredited program.
• Home health experience preferred.
• Strong knowledge of evidence-based treatment techniques and best practices.

Responsibilities

• Evaluate patients’ physical abilities and functional limitations in a home health setting.
• Develop and implement individualized treatment plans to restore function, improve mobility, and alleviate pain.
• Provide therapeutic interventions including exercise, gait training, manual therapy, and balance retraining.
• Educate patients and caregivers on home exercise programs, safety techniques, and adaptive strategies.
• Monitor patient progress and adjust plans as necessary to achieve optimal outcomes.
• Accurately document evaluations, treatment sessions, and progress notes using digital platforms.
• Collaborate with physicians, nurses, and other therapists to optimize care.
• Conduct home safety assessments and recommend strategies to prevent falls or re-injury.

Requirements:

• Active Physical Therapist licensein the state of Texas (TX)
