Actify Home Care is seeking a dedicated Physical Therapist (PT) to provide exceptional care to our geriatric clients in Broward County, FL. We are a clinician-owned & operated agency that values compassion, timeliness, and clear communication. Our goal is to improve lives by providing personalized in-home therapy that enhances mobility and quality of life for seniors.

What You’ll Do:

1. Deliver personalized, in-home therapy to improve mobility and quality of life for seniors.
2. Collaborate with patients, families, and our care team to ensure seamless treatment plans.
3. Maintain clear communication with all parties involved in patient care.

• Competitive pay: $65 per visit, $70 for evaluations/discharges

Requirements:

• Licensed PT in Florida with preferred geriatric or home health experience
• Strong organizational skills and punctuality
• Excellent verbal and written communication skills
• Driver's License and CPR/BLS Certification required
• Spanish speaking skills are a plus

Additional Details:

• Immediate openings for full-time cases with varying complexities
• Coverage areas include east of I-95 in Pompano Beach, Fort Lauderdale, Oakland Park, and Dania Beach
• Work location: On the road
• Part-time, PRN, and Per Diem positions available with benefits including competitive salaries and referral bonuses
• Must be able to relocate to Broward County if not already residing there
