WSi Healthcare Personnel is seeking a compassionate Home Health Physical Therapist to provide one-on-one rehabilitation in patients' homes in Colorado. You will evaluate mobility, design evidence-based care plans, and deliver therapeutic interventions to help patients regain independence.

You'll collaborate with an interdisciplinary team, educate patients and caregivers, and document progress in the EMR. The role offers a supportive culture with training and opportunities for career growth

Responsabilidades

• Deliver one-on-one home health physical therapy sessions in patient homes.
• Assess mobility, balance, endurance, and functional abilities.
• Develop and implement evidence-based care plans.
• Educate patients and caregivers on exercises and safety.
• Document care and progress in EMR and coordinate with the interdisciplinary team.
