Job summary

WSi Healthcare Personnel seeks a compassionate Home Health Physical Therapist to deliver personalized rehabilitation services in patients’ homes. You will evaluate functional mobility, develop evidence-based care plans, and provide therapeutic interventions to help patients regain independence.

Collaboration with an interdisciplinary team, patient and caregiver education, and EMR documentation are integral to the role.
