Full-Time Physical Therapist - Home Health Care

BAYADA Home Health Care is currently seeking an experienced Full-Time or Part-Time Physical Therapist to make home care visits to patients in Senior Living Communities (Independent Living and Assisted Living) in Centerville, Hyannis, and Yarmouth. Prior home care experience is preferred but not required. As a home health Physical Therapist, you will be an integral part of a multi-disciplinary healthcare team providing skilled nursing and rehabilitative care to help clients maintain their independence.

Responsibilities include:

1. Making visits to clients in their Senior Living Communities and homes.
2. Performing diagnostic tests and measurements, including mobility, joint range, transfer status, stability, ambulation patterns, muscle strength and endurance, balance testing, and safety assessments.
3. Developing and implementing individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
4. Assessing and revising the Physical Therapy care plan regularly, collaborating with nursing in the multidisciplinary care team.
5. Educating clients, family members, or representatives in rehabilitative care and activities to promote health, safety, and independence.
6. Documenting observations, interventions, and evaluations accurately using a state-of-the-art touchpad tablet.

Qualifications include:

1. A current Massachusetts Physical Therapist license.
2. Recent clinical experience as a licensed Physical Therapist.
3. Ability to travel to assigned cases.
