Full-Time Physical Therapist - Home Health

BAYADA Home Health Care is currently seeking a Full-Time Physical Therapist to provide exceptional care to adult and geriatric patients in their homes in Boston. Prior home care experience is not required. As a home health physical therapist, you will be an integral member of a multi-disciplinary healthcare team providing skilled nursing and rehabilitative care, helping clients maintain independence.

Responsibilities include:

1. Making visits to clients in their homes
2. Performing diagnostic tests and measurements, such as mobility, joint range, transfer status, stability, ambulation patterns, strength, endurance, balance, and safety assessments
3. Developing and implementing individualized care plans, including manual therapeutic exercises, gait training, and balance interventions
4. Assessing and revising the care plan, collaborating with nursing staff
5. Educating clients and family members in rehabilitative care to promote health and independence
6. Documenting observations, interventions, and evaluations using a tablet

Qualifications include:

• A current Massachusetts license as a Physical Therapist
