*Job Description:*
We are currently seeking an experienced *Travel Physical Therapist (PT)* for a *home health contract assignment*. This position is ideal for a licensed PT with strong clinical skills, who is confident working independently and thrives in a high-performance, fast-paced environment. You must have recent experience with *OASIS-E (post-Jan 2023)* and be proficient in *Homecare Homebase (HCHB)* with consistent productivity.

*Requirements:*
* *Active PT License*
* *Current BLS Certification*
* *Minimum 2 years of recent home health experience*
* *Proven experience with HCHB (Homecare Homebase)* – required
* *OASIS-E documentation experience (must be after Jan 2023)* – required
* *No employment gap exceeding 1 year*
* *Same-day documentation completion*
* Must meet *productivity standards of 5–7 points/day (26–28 points/week average)*This may translate to 3–7 patients/day depending on visit type
* Approx. *20–25 patient visits/week*
