Job summary

HealthCare Support Staffing, Inc. is seeking a Home Health Physical Therapist in Aurora, CO to provide skilled, one-on-one in-home therapy and evaluate mobility, safety, and function to help patients recover independence.

The role is field-based with a Mon-Fri 8am-5pm schedule and requires CAPTE-approved Master's in Physical Therapy, Colorado license, at least nine months' experience, and a valid driver's license.

Qualifications

• Minimum nine months experience as a physical therapist.
• Master's degree in physical therapy approved by CAPTE accredited program.
• Currently licensed to practice as a physical therapist in Colorado.
• Must be a licensed driver who can travel to all business locations.

Responsibilities

• Provide skilled in-home physical therapy care, evaluating mobility, safety, strength, balance, and functional abilities, then creating personalized treatment plans.
• Work independently in the field while collaborating with the care team, educating patients and families on exercises and safety techniques.
• Typically seeing 3-6 patients per day depending on visit type.

As a Home Health Physical Therapist, you'll have the opportunity to provide personalized, one-on-one care to patients in their homes.

Daily Responsibilities

• Provide skilled in-home physical therapy care, evaluating patients' mobility, safety, strength, balance, and functional abilities, then creating personalized treatment plans that help them recover independence after illness, injury, or surgery.
• Work independently in the field while collaborating with the care team, delivering one-on-one therapy in patients' homes, educating patients and families on exercises and safety techniques, and monitoring progress to ensure the best possible outcomes.
• Typically seeing 3-6 patients/day depending on the type of visit.

Required Qualifications

• Minimum nine months' experience as a physical therapist
• Master's degree in physical therapy approved by CAPTE accredited physical therapist education program
• Currently licensed to practice as a physical therapist in Colorado
• Must be licensed driver who can travel to all business locations
