Job summary

Your Health in the Aiken area is seeking a Primary Care Occupational Therapist to evaluate patients in homes, ALFs, and ILFs, develop individualized treatment plans, and supervise COTAs. You will collaborate with care teams and use telehealth tools to coordinate care.

This full-time role offers a salary with a monthly bonus and a company vehicle allowance, supporting high-quality patient outcomes and commitment to community health.

Qualifications

• Master’s or Doctoral degree in Occupational Therapy from an accredited program.
• Current state license to practice as an Occupational Therapist in good standing.
• Minimum of two years of clinical experience, preferably in home health or outpatient setting.
• Proficiency with Telehealth and EHR systems.
• Excellent communication and interpersonal skills.
• Ability to work independently and within a multidisciplinary team.
• Strong organizational and time management skills.

Responsibilities

• Conduct comprehensive evaluations of patients’ physical, emotional, and social conditions.
• Identify patient needs and set realistic therapy goals.
• Develop individualized treatment plans to improve daily living and work skills.
• Adapt treatment plans based on progress and feedback.
• Supervise and guide COTAs; review plan implementation.
• Utilize telehealth to collaborate with COTAs and professionals.
• Participate in care team meetings and coordinate care.
• Communicate patient outcomes and program improvements to management.
• Facilitate provider visits to ensure timely communication and alignment on plans.
• Document care management activities and maintain continuity of care.
• Educate patients and families on therapy goals and strategies.
• Provide training on adaptive equipment usage and daily function.
• Maintain accurate documentation of evaluations, plans, and progress.
